Puffy under-eyes usually aren't caused by new fat, but by the structures that hold the fat around the eye loosening so the fat is pushed forward.

Main causes

  • Weaker supporting structures — the membrane and ligaments around the eye fat loosen with age and the fat bulges forward
  • Genetics — some people have noticeable under-eye bags even when young
  • Loss of skin elasticity — as under-eye skin thins, bulging fat shows more
  • Volume loss nearby — a hollow forms below the bulge, making the difference in contour look greater

How it differs from temporary puffiness

Puffiness after poor sleep, salty food or alcohol goes down with time, whereas bulging fat is usually visible all the time, morning and evening.

Ways to address it

Lifestyle care can reduce puffiness, but it can't put bulging fat back. If fat is the main cause, a consultation about fat repositioning or lower blepharoplasty is an option.

Frequently asked questions

Can filler fix it? Filling the hollow can reduce the contour difference, but with a large fat bulge there may be limits. It is judged at the consultation according to your condition.
